The above may have been Jacob/Jankel's own translation, per a handwritten note that is with the passport.
A later handwritten note has this version:

? Pilyneit Clestnoya ?
Region
Kudagskov ?
The handwriting on the translations themselves is difficult to decipher, given a lack of familiarity with placenames.
Jacob, by the way lived in Svisloch/Sislevich (now Belarus), if that is of any help.
