I would like to get a translation of the document, especially the handwritten area. Who was covered by the document? What characteristics of Mendel (my great grandfather) were documented?

This passport documents my grandfather's family's exit from Romania. It is a copy provided by my 2nd cousin. The image is a screen shot from a PDF - the instructions didn't list PDF as an acceptable file format.
