Dear Doctor

In response to your letter from 17th this month I kinda inform that I was severely wounded by Gestapo [2] in march 1944 in village Skała nad Zbruczem. After expulsion of Germans from Skała I went to Łosiacz (powiat Borszczow), where I was treated for approximately two weeks by Russian doctors. After that I went to Borszczow, where I stayed from April 1944 until 1945 treated by you, both in hospital and in your private practice. I would also like to remind you that in 1945 you; issued medical certificate for me, and on this basis I was exempted for 14 days from railroad work as tinsmith (I’m a tinsmith by profession).
In September 1947 I moved to Canada, and between 1946 until leaving I was in Stanisławów.
For clarification, and reminding details I attached letter [3] from 27th September … (indecipherable year) and medical certificate issued by dr Boretski living in Montreal.
I was born in 1902
Due to above mentioned wounds I am still, after this long period of time unable to work hard [4].
I kindly request fast delivering me medical certificate. Only after receiving your certificate I will try to file for pension to German authority.
After receiving your bill I will send you all dues related with issuing this certificate.

[1] dr in Polish is abbreviation for scientific title doktor and medical profession, in second part it’s lekarz, so doctor or physician is best translation, there’s no indication of type of doctor
[2] no further details
[3] due to hidden parts of words and strange syntax I can’t tell whose letter was attached
[4] from the context it means hard physical labour
