passed away on the 26th of Tishrei, 5751 (year since the creation of the world). Last five letters are the initials of a verse "May his soul be bound in the bundle of life," a direct quote from the Book of the Prophet Samuel / Shmuel A, 25: 29, that is traditionally engraved on Jewish tombstones.

Observations:
Reb does not mean Rabbi it is a respectful way of referring to a man like saying Mr.
Anshei Sfard does not mean that the Congregation was of MIddle Eastern heritage, but that the order of the prayers followed the Nusach Sfard Version as mandated by the Chassidic Rabbis of the last 300 years. It appears that the deceased was active in two Synagogues in his lifetime.
The 25th day of the Hebrew month of Elul in the year 5719, began on Sept. 27, Sunday evening after sunset and ended Monday night. If he died on Sept 27, than his passing was sometime Sunday evening between Sunset and Midnight.
