Rivkah is Hebrew for Rebecca.

'Reb' is an honorific; it does not mean rabbi.

Her father had a double name. The first part, Shmuel, is Hebrew for Samuel. The second part, Velvel, is Yiddish for wolf/little wolf. SOME men with Velvel as part of their religious name have a secular name of Wolf, Wolfgang, etc. I've also seen William.

The Hebrew date of death is a bit complicated to explain, but I'll do my best:
“Rosh Chodesh”, literally the head of the month, also known as the New Moon, indicates the start of a new month. It can be a one-day or a two-day event. In this case, it was two days, and the first day of Rosh Chodesh of the month of Elul was actually the 30th day of the previous month, which was the month of Av. Rebecca died on the "eve" of this day, meaning that the actual date of death on the Hebrew calendar was the 29th day of Av. In the year 5701, this date began at sunset on August 11th and ended at sunset on August 12th, 1950.

By the way, we are currently in the month of Av. If the deceased is someone important to you, and you would want to observe the Yahrzeit (anniversary of the date of death), for example by lighting a memorial candle or giving charity in their memory, the 29th of Av will begin this year at sunset on Friday, August 22nd, and end at sunset on the 23rd.
