It seems that this document has headers in Russian, and is filled in in Polish. The column with the child's name also appears to have Hebrew in it. Can you please help with translation of the headers and the row with 80 in the second column? I believe that this is the birth record of my great-grandfather. The record was notated on JRI-Poland, and it is from the Polish Archives.

Thank you!
