Here is all the useful information:

Time and Place of Record: Staszów, 26 (14 [in the older Julian calendar]) March 1851, 9 a.m.

Witnesses: Eyzyk DUNAJET[?], synagogue sexton, 49 years old; and Szymon BRAM, day-laborer, 48 years old; both Jewish, both residing in Staszów

Time of Death: 25 (13) March 1851, 8 p.m.

Decedent: Lejbuś CZAULSZTIK, Jewish, day-laborer from Staszów, 40 years old, son of Lejzor and Hana Czaulsztik, parents deceased

Surviving Relatives: wife Hana, son Aba, two daughters Marya and Haja Sura.
