Below is a translation. I am uncertain of the spelling of names after which I have placed a question mark in square brackets—this issue arises when names signed messily on the document:

Circumcision/Naming: 26 April 1912, Drohobycz, 8 Borysławska street

Name of Child: Izydor
Sex: Male
Status: Legitimate

Father: Meyer Mordche Adam, merchant here. According to marriage certificate from Lwów, 22 June 1904 No. 216, legally married with the child’s mother on 20 April 1904 in Lwów

Mother: Pesia Adam née König, living here, wife of Meyer Mordche Adam, daughter of (deceased in Lwów) Ettla König, ritual [religious] wife of the innkeeper Dawid Gross

Witnesses, Sandek, etc.: Hersch Hoffman[n], peddler here; Dawid Erlbaum[?], private official here
