This record is beautifully composed. Here is a translation of all the useful information. Please note that the date in parenthesis is in the Julian calendar, while the others are in the (modern) Gregorian calendar. Also, the ages of the parents are likely as of the time of the report (1865), not of the birth (1851).

Time and Place of Record: Nieszawa, (5) 17 December 1865, 3 p.m.

Declarant (Father): Szlama TOROŃCZYK, forest trader [perhaps meaning “logger”?], 49 years old, residing in Nieszawa

Witnesses: Berek ŁOWICK, baker, 52 years old; and Szmul BACHRACH, synagogue servant, 32 years old; both residing in Nieszawa

Name of Child: Layzer Icek
Sex: Male
Time and Place of Birth: Nieszawa, 28 December 1851, 10 p.m.

Mother: Brana née GLICKSMAN, 46 years old, wife of declarant

[Paraphrased] The act was read to the declarant and witnesses and signed by them and us. The father attributes the delay [in declaring the birth] to his ignorance of the regulations and his belief that the report he made at the time was sufficient.
