A translation please, including the printed part of the form. This was my great uncle who married into the our Kahn family. He was a disabled vet from WWI. His son and daughter escaped the Nazis, but sadly his wife did not. His son Rolf became a good friend of mine and my family in NYC.

Time and Place of Death: Wiesbaden, Biebricher Straße 5a, 12 April 1930, 4:30 p.m.

Declarant was informed of death through own cognizance and signed.

Pronounced, approved, and signed.

Civil Registrar: Zimmermann

Accordance with main register certified: Wiesbaden, 14 April 1930.
