This is the death record of my 2nd Great Grandmother.
I need help reading/translating just a few bits:

The deceased's:
-Birthplace
-Father's surname, first name, job & residence
-Mother's surname, first name
-Cause of death (if given)
-The reporter's (her husband Bernat Berkovics) occupation

Number 97. Dated in Barkaszó, 14 October 1904.
Informant: Bernát Berkovics, cobbler, residence Csomornya, whom the below-written registrar knows personally.
Deceased: Mrs. Bernát Berkovics born Zseni Grünvald, Jewish, household, residence Csomornya, birthplace Szürte, age 60 years.
Spouse: Bernát Berkovics.
Father: the late Lőrincz Grünvald, day-laborer in various branches, Szürte.
Mother: the late unknown.
Death: Csomornya, 13 October 1904, 5 a.m.
Cause: typhus.
Read forth, approved, and signed. Due to informant's illiteracy, supplied with his mark.

Barkaszó, Bereg county, now Barkasovo, part of Rivne, Ukraine. Jewish residents recorded in Mezőkaszony, now Kosyny, Ukraine.
Csomornya, Bereg, later Csongor, now Chomonyn, Ukraine. Jewish residents recorded in Mezőkaszony.
Szürte (earlier sometimes Szürthe), Ung county, now Strumkivka, Ukraine. Jewish residents recorded in Csap, now Chop, Ukraine.
