Date and Place of Death: Berlin at Potsdamerstraße 28 in the Karewski clinic, 27 June 1895, 7:15 a.m.

Declarant: Dawid Oppler, merchant, residing in Berlin Große Hamburgerstraße 18/19, identity recognized by medical death certificate.

The declarant declared that he was informed of the above-mentioned death through his own cognizance.
