Permanently living in village Svidovka and registered as an urban dweller of Kotev city society - Galperin Yukel [?] ben Avrum

[I can clearly read letter Yu (Ю) k (к), but then it get's a bit unclear. The only names that I know of that start with "Yuk" are Yukev, Yukel, and Yukim].

Header:
1. Family ID number [the smaller number is the family number, the larger one is individual number]
2. Indication whether the person is added to the list based on the census or order of the government
3. Name of the neighborhood, where the street or block is located
4. Nickname or family name, name and patronymic
