This constitutes the second page of probably the most important 5 pages of what I believe to be a re-constituted birth certificate for my late materan Grandmother Esther Heringman (nee Stein) found in the Wloclawek Archives. I believe this occurred after her return to Poland following her escape to Russia after the Nazi and Soviet invasion of Poland during WWII when the original records were destroyed and she petitioned the court to reconstitute them. I would like to know all family information therein - good, bad and indifferent. This is probably the most critical page with not only birth particulars on my Grandmother but her parents as well etc. I am also interested in the column headings including the written information conveyed in the boxes. I seem to note further references for some further follow-up archival searchess. Many thanks in advance.
