This is a Hungarian civil birth register filled out in a Slavic language, probably Czech or Slovak. (In 1921, the post-Trianon states had not yet had time to create, print, and distribute new register-books. Some rural registry offices were still using the Hungarian books a decade later.)

It appears to be a delayed birth registration for Berta, daughter of Brána Wiesel. The pre-signature remark is probably the court decree permitting the delayed registration.
