He passed away 15/16 Kislev 5732

'Reb' is an honorific; it does not mean rabbi.

I am not 100% sure of the precise date of death. It looks most like the 15th day of the month, but there is a slight possibility that it was the 16th.

The 15th of Kislev of the year 5732 began at sunset on December 2nd, 1971, and ended at sunset on the 3rd.

The 16th of Kislev began at sunset on December 3rd, 1971, and ended at sunset on the 4th

His father's name, Baruch (also pronounced Boruch) means blessed. Some men with this Hebrew name (but certainly not all) have a matching secular name of Benedict or something similar.
