Natural father: Májer Paiszikovics, Jewish, chandler, residence and birthplace Miszticze, age 41 years
Mother: Fáni Jozevics, Jewish, residence Miszticze, birthplace Gázló, age 32 years
Birth: Miszticze, 15 April 1902, 12 noon
Child: boy, Jewish, Mordhe
Remark: informant declares that he acknowledges the herein-registered child as his own, and he was present at the child's birth. In the pre-printed text the word "legal" has been crossed out and replaced with "natural".
Read forth, explained to the non-Hungarian-speaking informant in the German language understood by him, approved, and the illiterate informant made his mark.
