Comment: in the 1950's questionnaires were usually filled by representatives from Yad Vashem in peoples homes, and only signed by the witnesses. Though there's no represntative's signature on this document, the handwriting in the signature looks different from that in the name. The middle name is a surname -- either maiden or married.
