Please translate all the Hebrew so I can see if there are different spellings. Mother's name, Father's name, and spouse's name full name. My father said it was Alte but like Tuvia I come up with nothing.

The family name is Gold.

The first name is Tevye

The father's name is Ozriel/Azriel

The mother's name is Malkeh

I am wondering whether perhaps the person filling out the form did not understand what was being asked for, especially if he/she was only looking at the Yiddish, which is subject to misinterpretation.

The first word is Tevye. The second is Alte, which is a possible name but also means 'old'. The last word is Brevde. This MIGHT have been saying that the man's name was Tevye, and his wife's name was Alte and her maiden name was Brevde.

The test is all in Yiddish. It's clearly a document meant to be filled by both males and females, with "options" in parenthesis.

Line 7, translated from Yiddish says: "Name of husband or of wife and her maiden name".

I think the person who filled the POT was thinking: Husband:that's Tevye (a Yiddish pronunciation of Tuvya), wife and her maiden name: that's Alte Brevde [I would also search for Broide,]
