This is the marriage document of Lejzor Hochberg and Sura Gitla Zlotkowska. I am wondering if Zlotkowska is her birth name or was she married before? The marriage document for their son indicates that her birth last name was Judkewicz. Can someone please translate and tell if this document indicates whether she had been married before. Thanks much.

Miss Zlotkowski is 22 years old and is described as single
her parents' surname is not separately mentioned
I conclude that Zlotkowski is likely her surname at birth
Her parents are both deceased and she lives with unspecified family, and I can imagine that her parents could have died when she was very young and she could have been adopted by a family member with a different surname but then why is her parents' surname not mentioned or her own (same) surname at birth? Was her son's document you mentioned previously posted here?
