'Reb' is an honorific; her father was not a rabbi. He was, however, a Cohen, member of the priestly tribe tracing back, son to father, to the first priest, Aaron (Moses' brother).

The 17th of Cheshvan, 5734, began at sunset on November 11th, 1973, and ended at sunset on the 12th. The secular date on the stone is the 11th, which means that she passed away between sunset and midnight of that day.
