What is the purpose of this form? I'd like to know any biographical information contained in it, like age, relatives' names, occupation, etc.

Date 14 May 1969
A form sent to an address I can't make out, asking them to try to find the present address of Sofia Vladimirovna, date of birth 1900, father's name Mendel, since the Stern family from South Africa asked about her, and the bureau wants to contact her to further the inquiry as soon as possible.

Next to Comments is says: Id. card No. 6588515
