This is from the file of Sofia Yodaikin Vladimirova, from the Jewish Agency's Bureau of Missing Relatives. What is the purpose of this form? I would like a translation of each field. I would especially like to know if the month of arrival in Israel in 1960 is known, and what is the significance of the number 3/16 in the bottom left.

There are no details on the right side for town of birth, year of birth parents names.

Correction - this is apparently a response to recieved letter.

Date the form was filled: 19 May 1969

Printed statement on top right: "In response to your letter of" 14.May.1969 "I am informing you that the information is meant for me."
