'Reb' is an honorific; his father was not a rabbi.

The name of the deceased is preceded by an abbreviation that translates to "our teacher, the rabbi". This means that he had rabbinic ordination, not necessarily that he was the rabbi of a community or synagogue. I have found that this abbreviation is sometimes used in error, so it would be good if you can confirm his rabbinical training from a secondary source.

The date of death corresponds to April 5th, 1914.

Yaakov is Hebrew for Jacob. His father's name, Dov, means bear. Men with this Hebrew name sometimes -- but certainly not always -- have a corresponding secular name, such as Bernard or Barry.
