I would greatly appreciate a translation of this 1850 Death record from Polish. Please attempt a full (literal) translation and include all names, dates, ages and places.

Year 1850
It took place in Ostrołęka on the December 26/January 7, 1849, at 9 AM. There appeared Jew (orthodox = starozakonny) Chajm Wylozyn, a synagogue sexton, age 53, and Chajm Ejzenberg, a baker, age 64?, resident of Ostrołęka, and they declared that on the January 21/February 2nd?, at 2 P. M., died Juzko (Josko?) Mejerowicz Dzbanek in Siedlisko, a two-year old son of Mejer and Malka, spouses Dzbanek, residing in Siedlisko. Having convinced themselves personally (naocznie = with their eyes) ...? (I can't read this but it should be something like "of the passing") of the life of Juzek Mejerowicz Dzbanek of Siedlisko, this act was read to and signed by those appearing

P. S. I am sorry but I cannot decipher some words. The left margin not signed by the witnesses adds the year 1850 to the record. Also the place name is Siedlisko?
