[this is followed by two and a half lines of flowery praise that I will translate if it's important to you.

The name of the deceased was CHAYIM ASHER

His father was Shmuel.

They were both Levites, tracing their ancestry back, son to father, to Levi, one of the 12 sons of Jacob.

The Hebrew date of death was the 23rd of Tevet, 5694.

His father's name, Shmuel, is Hebrew for Samuel. This might have been his secular name, but this is not necessarily so.
