I recently found this image, the only primary record we've ever found for my father-in-laws grandmothers family in Poland - the birth record for Jakob Holauber.

Would appreciate it if someone could look over entry 56 and translate any text in Columns 3 (House Number = Synagogue? what does the header say?), 6, and 7 - there are some words mixed in with the names in those columns I don't understand...
