City of Ostrołęka. It came to pass in Ostrołeka on the 17th/29th day of September 1866 at 4:00 PM that there appeared [before us] the Jew Chaim Wyłoźny, sexton, age 71, and Lejb Śniader, synagogue employee, age 51, residents of Ostrołeka, and informed us that on the 16th/28th of September of the current year at twelve noon in Ostrołeka the Jew Dawid Zabielak died at age 25, the son of unknown parents, leaving behind his wife Szejna Zabielak. After verification of the death of Dawid Zabielak with our own eyes, this document was read out to the petitioners and it was signed only by Wyłoźny and ourselves, as Śniader does not know how to write [Polish].
