Place: Posad Kotsk.
Registration date: May 25, 1890 at 9 am.
Mendel Eisenman, 29 years old, a businessman selling goods ("torgovets"), an inhabitant of Kotsk Posad, appeared with a male baby, in the presence of two witnesses, Itsko Zilbergeld and Abram-Gerhko Gershenborn (both are homeowners, also inhabitants of Kotsk Posad), and declared that the baby boy was born on May 17, 1890 at 4 am in Kotsk Posad to a lawful wife of his (Mendel Eisenman's) Mnikha-Laya, nee Fridman, 28 years of age. Baby boy was given a name of Abram-Gersh at Brit Mila.

I translated the names of witnesses, because it is known that witnesses may be relatives of the family registering a birth. There is also a note that the baby's father and witnesses were illiterate and the document was signed by government officials.
