Request for declaration of the death certificate

I request the court in Warsaw to issue a declaration of the death certificate of Pinkus Altman son of Mendel and Chana nee Birnbaum, born on the 5th of July 1903 in Kowal, died in Warsaw during the german occupation on 30th December 1942.
For confirmation of these events please call a witness Klara Strzyk, living in Łódź, Zawadzka 22, whom I declare to deliver on the day of the hearing,
Please, conduct a hearing even in case of my absence.
Warsaw 24th may 1949
