She passed away the 28th of Iyar of the year 5669 [?]

May her soul be bound up in the bond of life.

As always, 'Reb' is an honorific; it does not mean rabbi.

I am not sure of my reading of the place of origin. It looks like Kaidonov or something similar.

The first vowel in her married name is faded; I know that you wrote Halprin, but it looks more like Helprin.

The 28th of Iyar of the year 5669 began at sunset on May 18th, 1909, and ended at sunset on the 19th.

There is an ever-so-slight chance that the year is 5689, which would make the secular date of death June 6/7, 1929. Do you know which year is correct?
