This is a birth record for Michal Apfelroth (first row); I'm hoping someone may be able to decipher the employment (?) information under the mother's name. I think her parents may also be listed?

I see the birth is recorded as illegitimate (perhaps Michal's parents had a religious ceremony only?), and suppose this is why the father, Wadje Apfelroth, is listed in the last column.

You'd have to sharpen the image quite a bit for this to become legible. The father column - column 6 - empty.
It does say nieślubne in column 7, meaning out-of-wedlock.
