Would much appreciate as much a translation into English as possible of the personal information, especially children's names, of this Page of Testimony. It would also be helpful for me to have a brief English translation of the pre-printed information in the numbered lines. Thank you!

13 Name of wife and her maiden name; age: as noted above; age: 1903
Name of husband; age: Yisrael Yaakov; age: 1901
14 Names of children up to age 18; their age; when and where they perished
Noah Bloch 17; Daugay 1940
Witness' details identical to those in VM 94144 except for relationship. The word "her brother" is erased, replaced by "their uncle" of Rivka Kaleka Bloch.
I realize this doesn't fit VM 94144, where "his brother" is written as clearly as one can want.

A stamp on the top of this document indicates it's a copy – maybe that can explain the rather strange correction – the copy was there to create a separate POT the children from that given for the mother.
