'Reb' is an honorific; it does not mean rabbi.

Only the Hebrew year is given here, even in the English. The secular year was 1939.

You asked about the "unusual form of the 'kof' character at the end of the second line".

This is actually a special symbol found after the date on many tombstones. It is a 'glyph' composed of the letters Lamed, Fay, and Kuf, superimposed upon each other. (This glyph was presumably created in order to save space and/or money, as only one "letter" has to be engraved instead of three.) These three letters can often be found individually engraved, with a double quotation between the Fay and the Kuf, to indicate that this is an abbreviation. The abbreviation represents the words "LiFrat Katan", which translates roughly to "according to the small details". This means that the Hebrew year that precedes this abbreviation is missing the character(s) that would represent the thousands digit in the year, which is implied. In other words, the year on this stone actually reads 699, not 5699.
