Attached is the birth record (akta 18) of my second great-uncle Pejsak Rzetelny, the son of Chaim Zyskind Rzetelny (whose name I believe I can decipher in the record) and Golda Chaja Rzetelna (nee Brzytwa).

Младенцу при религиозном обряде даны имена Алтиа-Сура.
Настоящий акт явившемуся и свидетелям прочитан, ими и свидетелями подписан; отец неграмотный.

Appeared Chaim Zyskind Rzetelny, aged 40, a *talasnik* [maker of prayer shawls ?], a resident of the town of Kałuszyn, and—in the presence of witnesses Szmuel Lejb Biblarz, aged 50, and Szmuel Boruch Petelbaum, aged 71, *szkolniks* [synagogue attendants] residing in the town of Kałuszyn—presented to us a female infant, declaring that she was
born in the town of Kałuszyn on April 10, 1889, to him and his lawful wife Chaja Golda, née Brzytwa, aged 30.
During the religious ceremony, the infant was given the names Altia-Sura.
This document was read to the declarant and the witnesses and signed by them and the witnesses; the father is illiterate.
