Spouse: Lázár Jakubovics
Parents: Salamon Sehn, Sáli Szrulovics
Place of death: Erzsébet királynő ("Queen Elizabeth") street number 9 (presumably in Sátoraljaújhely)
Cause: meningitis
Pre-signature remarks: birthplace Pichnye. Entered based on written report per (registry instructions number something-or-other that I'm not going to try to extract from this handwriting).

Nagyzalacska, Ung county: now Veľké Zalužice, Slovakia.
Pichnye, later Vendégi, Zemplén county: now Pichne, Slovakia.
