This is Jewish birth record No. 14 from Widawa, Poland, from 1914, for Izrael Icek Lipszyc (also spelled Lipshitz), son of Szmul/Szmuel Lipszyc and Rajzla/Raizel Zyscholc.
I would especially like to know the exact date of birth of Izrael Icek, including both dates if the record uses the Julian and Gregorian calendars.
I am also trying to determine whether the record says that Izrael Icek was a twin, or contains any wording that would indicate a multiple birth. His brother Chil/Yechiel Lipszyc is also recorded elsewhere as having been born in 1914, so this distinction is important.
If possible, I would appreciate a translation of the relevant portion of the record and clarification of the birth date and any twin notation

Отец ребенка : Шмуль Левек Липшиц, торговец, живет в этом посаде, 40 лет.
Мать: Райзля, урождённая Зысхалец (З.Ы.С.Х.А.Л.Е.Ц.), 42 года.
Ребенок: мальчик Израиль Ицек, родился в этом посаде, из близнецов (!) ,16/29 апреля 1914 года.
При подписании акта отец ребенка заявил, что неграмотный.

Mother: Rayzlya, née Zyskhalets, 42 years old.
Child: a boy, Izrail Itsek, born in this settlement; one of twins (!); born April 16/29, 1914.
Upon the signing of the record, the child's father stated that he was illiterate.
