Four fields, top to bottom:
- parents' residence
- day and year of BIRTH in words, Hebrew date
- place of birth
- day and year of the BRIT in words, Hebrew date

Please read the two Hebrew dates. My reading is 28 Elul 5663 for the
birth and 6 Tishrei 5664 for the brit - eight days apart, which is what
the brit requires - but I am not confident of the month words.

Context: the Lithuanian side of the same document gives 7 September 1903
and 14 September 1903. If the Hebrew dates are as I read them, those civil
dates are Old Style (Julian) and the Gregorian equivalents are
20 and 27 September 1903.

I read the dates of 28 Elul and 6 Tishri. I don't see the Hebrew years.

However, the Brit seems to have been postponed by a day. The Brit is traditionally on the 8th day of the child's life, thus 7 days after birth. Barring a medical condition that would make it unsafe for the baby, the circumcision is on the same day of the week as the birth, one week later. (One exception: if the baby is born by c-section on the Sabbath, his brit is the following Sunday).

I am returning to correct my previous error.

As the poster correctly points out, the month of Elul has only 29 days, and therefore the 6th of Tishri was seven days after the 28th of Elul (not eight, as I had written).
