It's definitely Dólezsák, not Polatsek. For the first letter, compare Dávid in the last line of family 48 and Pepi in the last line of family 49.
("Zabolcs" is of course Szabolcs, and "Tsaba" is Csaba, more specifically Hejőcsaba, which has now been absorbed into Miskolc.)
