Here is a translation. (This is all Polish, not Russian, as it is from after the First World War.)

No. 2296

Łódź, the 16th of December 1938. Today, the following adults appeared before the undersigned Registrar of Civil Status: Moszek PANTEL and Hilel CHARMAC, employees of the local religious community; and declared that Abram BORENSZTAJN, 32 years old, unmarried, a tailor, born in Łódź, a permanent resident of Łódź, Jewish religion, son of the married couple Dawid Chaim BORENSZTAJN and Etta née LEWKOWICZ; died in Łódź on the 15th of December of the current year at 07:00.

After [the Registrar was] personally convinced of the death of Abram BORENSZTAJN, this act was read, accepted, and signed.
