the woman SARAH daughter of Reb

Ephraim, who passed away on the
3rd day [of the Jewish week], the 4th day of the month of Av 5681

May her soul be bound up in the bond of life.

'Reb' is an honorific; it does not mean rabbi.

Ephraim [ef-RAH-yim] is a Biblical name.

The "third day of the Jewish week" runs from sunset on Monday night to sunset on Tuesday night.
