I believe this is a Polish birth certificate from Lodz. I would like to know the date of birth, and ideally the rest of the translation for Chaskel Mordka Mordkowicz.

He is the one on the right hand page (son of Mendel and Rywka)

I got the image from the Lodz town volunteer at JRI Poland

Łódź, 15 May 1922. Today, Mendel MORDKOWICZ, tailor aged 25, permanent resident of gmina (commune) Janki, Radom county, member of the religious community in Łódź, appeared before the undersigned Civil Registry Officer, and in the presence of witnesses Ezryel POWOGROD, aged 37 and Jozef KAGAN, aged 52, both merchants in Łódź, stated that a male child had been born in Łódź on 28 March 1922 at 3 o'clock in the afternoon to his wife Rywka, born SZWARCBERG, aged 23.

The child was named Chaskiel Mordka.

The act was read and signed by those present.
