I downloaded this birth record from the Polish State Archives website, via JRI-Poland. It is the 1906 birth record of Froim Kociak, son of Chaim Ziskind. I would appreciate a full translation.

Состоялось в городе Ломже 4 сентября (22 августа по старому стилю) 1906 года в 9 часов утра.
Явился лично житель деревни Гаць — временно проживающий в городе Ломже Хаим- Зыскинд Фроимович Коцяк, 26 лет от роду, в присутствии свидетелей, жителей города Ломжи:
• Абрам Давидович Шейпкоп, школьника (синагогального служителя), 54 лет (похоже на то, что это отец жены Хаима);
• Янкель Цетюк, торговца, 53 лет.
Он предъявил нам младенца мужского пола, родившегося в городе Ломже 28 августа (15 августа по старому стилю) этого года в 3 часа дня от законной его жены Малки Абрамовны Давидовны , урожденной Шейпкоп, 27 лет от роду.
Младенцу этому при обрезании дано имя Фроим. (в честь отца Хаима).
Акт этот явившемуся и свидетелям прочитан и всеми подписан:
• Х. З. Кочак
• А.Д. Шейпкоп
• Я. Чешлюк
(обратите внимание на то, что подписи отличаются от написание фамилий клерком, который записывал акт).

It took place in the city of Lomza on September 4 (August 22, Old Style), 1906, at 9 a.m.
A resident of the village of Gats, who was temporarily residing in the city of Lomza, named

Chaim-Zyskind Froyimovich Kotsyak, 26 years of age, appeared in person in the presence of witnesses,
residents of the city of Lomza:
• Abram Davidovich Sheikkop, a schoolboy (synagogue attendant), 54 years of age
(it appears that this is the father of Chaim's wife);
• Yankel Tsyutyuk, a merchant, 53 years of age.

He presented us with a male infant, born in the city of Lomza on August 28 (August 15, Old Style), this year, at 3 o'clock in the afternoon, to his lawful wife, Malka Abramovna Davidovna, née Sheikkop, aged 27.
This infant was named Froim during its circumcision. (in honor of his father, Chaim).
This act was read to the witness and signed by all:
• H. Z. Kochak
• A.D. Sheikkop
• J. Cieszlyuk
(note that the signatures differ from the spelling of the names by the clerk who recorded the act).
