Attached is a document from the 1936 case of Hero of the Soviet Union Semyon Samuilovich Levin, my second great-uncle, in the Communist Party (courtesy of RGASPI). Would appreciate your assistance in its translation.

(Side note: my final attempt to contact RGASPI was made thanks to Mrs. Svetlana. I could not obtain the 1954 case as well, since Levin's birth record was required, and I am currently unable to obtain it.)
