The Hebrew name Rachel is pronounced rah-CHELL in modern Hebrew/Sephardic pronunciation, and RAW-chell in traditional Ashkenazic. The ch is pronounced as in Chanukah or Bach.

Reb is an honorific.

Reuven [reh-OO-vayn] is Hebrew for Reuben.

The 14th of Tamuz, 5754, corresponds to June 22nd/23rd, 1994.

(By the way, if this is someone important to you, and you would like to mark the Jahrzeit/Yohrzeit [anniversary of the date of death] by, for example, lighting a memorial candle or giving charity in her memory or doing an act of kindness, etc., the 14th of Tamuz is coming up soon.
