Reb ELIYAHU son of Reb Avraham

He passed away 22 Tishri 5726

May his soul be bound up in the bond of life.

'Reb' is an honorific; it does not mean rabbi.

Eliyahu is Hebrew for Elijah. Avraham is Hebrew for Abraham.

The 22nd of Tishri, 5726, corresponds to October 17th/18th, 1965.

Please keep in mind that having the religious name of Avraham does not definitely mean that his secular name was Abraham. It could have been something similar, but might have been something completely different.

A very popular Americanized name upon arriving in this country could have been ABE or ABBIE. But, as stated by the previous responder, it could have been anything either sounding like Abraham, starting with a letter "A" or not.
