It's pretty accurate. I'd add that the father and the two witnesses were all cobblers, and Abram was 38, not 39. There is no age indicated for Herszek PASTERNAK.
The mother's maiden name/patronymic is z Herszków (either her father was Herszek or Herszek was her maiden name) not Hersznów and she and the father were spouses.

4. Andrzejewo. It took place on the fourteenth day of May in the year of eighteen sixty-five, at the hour of four in the afternoon, in the town of Andrzejewo. Appeared personally the Jew Herszek Jankielowicz Chalupowicz, a shoemaker, twenty-seven years of age, the father of the deceased, and the Jew Herszek Pasternak, a shoemaker, and also Abram Chalupowicz, a shoemaker, both thirty-eight years of age, both residing in Andrzejewo, and stated that the Jew Iciek, one and a half years of age, the son of the appearer Herszek Jankielowicz Chalupowicz and Hana Chalupowicz the daughter of Herszek, a married couple, had deceased earlier that day, at the hour of four in the morning, here in Andrzejewo. After it was visually ascertained that Icek Jankielowicz Chalupowicz had deceased, this act was read aloud to the appearer and witnesses and subsequently signed, albeit only by me as the appearer and witnesses could not write.
