This is a tragic notation:

Rosh Chodesh Elul 5699 (This date occurred between sunset August 14th and sunset August 16th, 1939. I can't be more precise, because there were two days of "Rosh Chodesh Elul").

Moshe Tzvi of blessed memory, son of Yaakov Yosef (there seems to be more on this line, but it is cut off at the left edge). Moshe is Moses. Tzvi is Hebrew for deer; some men with this name had the nickname of Hersh. Yaakov is Jacob. Yosef is Joseph.
