I would like a translation of the Hebrew/Yiddish name of the young woman who died. On Jewishgen, her name is listed as "Fale?", from the German side of the record.
Any information about the names of the father or mother of the young woman would also be useful. Thank you.

The young (unmarried) woman's name might be read on the German side as Fale or Fele (the second letter is smudged). I lean towards Fele, pronounced FAYleh. This name exists in my (German-Jewish) family, so I have some confidence in my reading, reinforced by the Hebrew side of the document.

Her mother's name is not given.

Her father was a slaughterer/butcher named Herz Mottel Schawelowitz (German w's are pronounced like English v's).

The Hebrew side includes the name of the father's father, but I cannot make it out. It might begin with an M.

Translation of the Hebrew

The maiden [not previously married] Irleh daughter of the butcher [*not* slaughterer] Hertz son of Mordeccai Shabelovitz [or Shavelovitz]

[Comment]
A butcher sells meat. A slaughterer is a person who has studied the laws of halachic slaughter and was tested and ordained. The Hebrew term is שוחט or שו"ב neither of which appears in this description.
