Do you have a clearer photo (or at least a higher-resolution image?).

The text is very fuzzy and becomes fuzzier if I zoom in.

All I can read with any confidence is the first part of his name: Chaim [chah-yim] and the second part: Yehuda (Judah). These are followed by what might be Karali or similar.

The following line might be his town of origin, as it appears to begin with "from the city of ...", followed by what might be Tash____.

I'm sorry that I can't read more. If you have a higher-resolution photo, please let me know.

I tend to think the city referred to is Kashau, or today known and spelled as Kosice. (The Kosice cemetery does not seem to have anyone with this name of KARALI (probably pronounced more like KARAY.)
